Over the past 15 years working in the internal audit arena I have seen a growing interest in the topic of Root Cause Analysis (RCA). My involvement in the topic has evolved from using it as part and parcel of a “lean auditing” approach, to running RCA workshops and consulting advice across Europe. I was also responsible for helping the IIA UK to up-grade its guidance on Root Cause Analysis (published in 2015). Over the past 12 months I have delivered open and in-house courses in the UK, France, Norway, Switzerland and even Mauritius!

James: We’re going to talk about culture, and behaviour, and how you would know that you have a good culture. Can you please take us through some of the thoughts you have about how to approach this based on your own experiences in HR?
Jenni: In my experience culture is significantly influenced by the strategy of an organisation and its leadership; these are the key transformational influencers. The culture is also affected by the external environment, so what industry sector the organisation is in, what markets and its competitive environment etc.
Then how you measure it, and how you see it, how things work around here, is best evidenced by the observable behaviours, what goes on around here. This can also be complemented by an annual employee survey.
As mentioned before, the culture will be influenced by the CEO and the direction that he or she sets, in terms of objectives and values. When done well, expected values will articulate the sorts of behaviours they want to see; but this will – of course – be influenced by day-to-day management behaviour and the behaviour of the staff around you, creating sub-cultures. Sub-cultures will comprise of factors such as “Am I fairly rewarded”, “Am I recognised”, and “Do I feel my views are heard”? Continue Reading

James: Thanks for agreeing to be interviewed Ralph. Can we start by talking about the idea of a healthy culture and less healthy culture and how we might measure this?
Ralph: Let me start of by saying that there’s an awful lot of stuff said about culture that needs to be understood in a broader context. So what we mean by culture, and a good culture, will often be put it in the context of the society you are in, as different societies have different cultural preferences.
In addition, there is often a link between the personality of senior leaders and the attributes they look for in a company culture. However, 1) I am not sure they understand in depth what culture means, and 2) I am not sure they understand what sort of organisational culture is actually going to be helpful to performance in a sense broader than pure financial results. Put another way, the people in power in an organisation often want to promote a culture that suits them for personal reasons, and I don’t mean in any corrupt way, but often they seek something that they are comfortable with. I contacted [James] for advice as to how Assurance Mapping techniques could be applied in my own authority. Not only were his ideas very constructive, his approach was very professional yet carried out in a friendly, easy going manner which greatly helped my understanding of the issues involved. James was then always happy to deal with any follow up queries I had and he helped to fine tune and discuss the approach with the rest of my team who similarly very quickly formed the same positive view of James’s style and manner. We have recently introduced this new approach into my authority and the results and comments to date have been very encouraging, thanks in no small way to James’s advice and assistance. I would have no hesitation recommending James to anyone.

James had conversations with us and read through our existing methodology documentation, before proposing an agenda and associated materials for a workshop. James ran a very successful lean auditing workshop for us, working in close collaboration with my team and I. It was instrumental in surfacing a number of opportunities to improve our team ways of working and has helped us make improvements in our productivity and value adding contribution. James is to be highly recommended.

James Paterson's book [Lean Auditing: Driving Added Value and Efficiency in Internal Audit] provides the reader with an excellent guide to lean auditing. But not only that it also provides the reader with an excellent framework for how to lead an Internal Audit function and how to develop strong working relationships with internal audit's key stakeholders.

A book for progressive auditors and the next generation. An excellent read. James takes a profession that could be seen as dull and boring and brings colour to it. This a book for progressive auditors and the next generation - individuals who really see the value in what they are doing and how to get the absolute maximum from the resource. The courses are given on a structured way and very clearly. The underlying documentation is useful and available in pdf. format. There is a lot of interaction with the students. The students can contribute to the choice of topics they are interested in.
Many of our delegates have attended the CIPFA Audit Conference for a number of years, making them a very discerning and often hard-to-please audience. James’ session on the challenges facing heads of internal audit today was described by many attendees as one of the best of the conference, particularly in terms of relevance and focus.

I found the workshop that James delivered to be very helpful and very informative and think that it will benefit us all moving forward. We are currently in the process of moving to using the most recent version of Galileo where the focus will be more on risk and thus a lot of the knowledge gained at the course can be put into practice. I think that the course would be very beneficial to others as well.
James is always an engaging tutor. I came along with a view that I already had a fair knowledge about culture, I thought this training course would be a good way to supplement my knowledge and improve understanding. Well, improve understanding it did, but James did this by blowing away any pre-conceived ideas I had. James ensured we were all engaged, and through asking some really good questions about our experiences within the culture area, he was able to break down my assumptions and show me that my knowledge only just scratched the surface. This meant that by the end of the day, my ideas of culture had been completely broken down and then rebuilt.

Your workshop differed so much because it was shot through with pragmatism and delivered in a refreshingly jargon free manner. Most of the stuff we do as auditors is common sense and there is no need to dress it up as pseudo-science hiding behind meaningless management speak – yet too many on the speaking circuit do just that. I thought you got to the heart of the matter in a very clear and direct way; you helped crystallise some thoughts I have been having recently and offered new insights that I was not familiar with ……so a thoroughly good session, one of the best I have been to for a long time.
